 To submit application using on-line form:

  1. Download the appropriate Application
  2. Open the Application in Adobe Acrobat Reader
  3. Select "Fill & Sign" option
  4. Enter all fields that apply to your application by clicking on a field (The Tab key may be used after a field is clicked)
  5. Print the application
  6. Have form notarized
  7. Scan the form and all associated documents into a file and save as an Adobe Acrobat Reader (.pdf) file.
  8. Click Here to fill out the online form
  9. Enter required on-line form fields
  10. Add your scanned document to the on-line form
  11. Click on the submit button

 

To submit application using Adobe Acrobat Reader:

  1. Download the appropriate Application
  2. Open the Application in Adobe Acrobat Reader
  3. Select "Fill & Sign" option
  4. Enter all fields that apply to your application by clicking on a field (The Tab key may be used after a field is clicked)
  5. Print the application
  6. Bring the complete notarized form to our address

 

To submit using a Typewriter:

  1. Open the Application
  2. Print the application
  3. Fill out the application using a typewriter
  4. Bring the complete notarized form to our address

 

Note: New applications must be submitted thirty (30) days prior to the meeting and accompanied with the required $500 fee. Renewal applications must be submitted thirty one (31) days prior to the renewal date for the license and accompanied with the required $500 fee.
