Applications
To submit application using on-line form:
- Download the appropriate Application
- Open the Application in Adobe Acrobat Reader
- Select "Fill & Sign" option
- Enter all fields that apply to your application by clicking on a field (The Tab key may be used after a field is clicked)
- Print the application
- Have form notarized
- Scan the form and all associated documents into a file and save as an Adobe Acrobat Reader (.pdf) file.

- Enter required on-line form fields
- Add your scanned document to the on-line form
- Click on the submit button

- Bring the complete notarized form to our address
To submit using a Typewriter:
- Open the Application
- Print the application
- Fill out the application using a typewriter
- Bring the complete notarized form to our address
Note: New applications must be submitted thirty (30) days prior to the meeting and accompanied with the required $500 fee. Renewal applications must be submitted thirty one (31) days prior to the renewal date for the license and accompanied with the required $500 fee.
